For children 2–5 years old (toddlers and preschoolers), neem leaf extract is generally low risk when used on the skin in normal product amounts. Most safety concerns come from a few animal or lab studies using high doses, not from normal topical use.
If you want to use a product that contains neem leaf extract on a 2–5 year old: do a small patch test on the inner forearm and wait 24 hours for any reaction; avoid using it on broken or irritated skin; do not let the child lick or swallow the product; use sparingly and avoid applying to large areas every day for long periods; stop use and speak to your child’s doctor if you see redness, a rash, swelling, or other skin changes.
There are a few scientific studies that found effects in animals or cells at high doses — for example a study linking neem leaf extract to lowered egg cell quality ("Neem (Azadirachta indica L) leaf extract deteriorates oocyte quality by inducing ROS-mediated apoptosis in mammals"), a study showing possible thyroid changes in mice ("How safe is neem extract with respect to thyroid function in male mice?"), and a lab study reporting mutagenic findings ("Genotoxicity of a crude leaf extract of neem in male germ cells of mice"). Because these findings come from animal or lab tests at high exposures, they do not prove harm from normal skin use, but they are why we recommend avoiding heavy, repeated, or long-term use on young children and avoiding ingestion. Also note there is low-level concern about environmental toxicity referenced in hazard labeling (EU GHS).
