Niacinamide is usually safe for toddlers and preschoolers when used in small amounts in skin creams or lotions. It helps skin look and feel better and rarely causes serious problems.
Use products made for children or gentle formulas. Do a small patch test on the inside of the arm first. Start with a small amount once a day. Don’t put it on broken or very red skin. If your child gets a rash, swelling, strong redness, or stinging, stop and talk to your child’s doctor.
A single animal study at very high doses reported tumor formation (Role of nicotinamide in DNA damage, mutagenesis, and DNA repair, 2010), but that study used much larger exposures than normal topical use. Industry safety reviews also note some data gaps and recommend limits on concentration and product types. Also, niacinamide can increase skin absorption slightly — avoid using it with many other strong products at the same time and avoid applications to irritated or sunburned skin.
